Waiver and Release of Liability

I declare that as the parent or legal guardian of the player registered on this form, hereby give my permission for my child  ("my child) to participate in the RVFA Ready Summer Camp (“the Soccer Camp”) from June 23rd 2025 to June 27th 2025.

 

In consideration for the opportunity for my child to participate in the Soccer Camp, I, for myself and on behalf of my child, hereby release and forever discharge RVFA Ready Soccer Camp, its officers, directors, agents, employees, and volunteers from any and all liability, claims, demands, or causes of action which may arise from or be in any way connected with my child's participation in the Soccer Camp, including but not limited to any injuries or damages suffered by my child during the Soccer Camp or while traveling to or from the Soccer Camp.

 

I acknowledge and understand that participation in the Soccer Camp involves inherent risks, including but not limited to physical injury, illness, or death. I hereby assume all risks and hazards incidental to my child's participation in the Soccer Camp, and I hereby waive, release, absolve, indemnify, and agree to hold harmless the Soccer Camp, its officers, directors, agents, employees, and volunteers from any and all liability, claims, demands, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child during the Soccer Camp or while traveling to or from the Soccer Camp.

 

I certify that my child is in good health and that there are no physical limitations to my child's participation in the Soccer Camp. I understand that I am enrolling my child in the Soccer Camp at my own risk and that RVFA Ready Soccer Camp, its officers, directors, agents, employees, and volunteers shall not be liable for any injury, illness, or death to my child or for any damages arising from my child's participation in the Soccer Camp.

By participating in the Soccer Camp, I, acknowledge and agree that the camp may take and use photographs and videos of me/my child for promotional purposes. I waive any claims and the right to inspect or approve the content, understanding that this release is granted without expectation of compensation.

I acknowledge that I have carefully read this Waiver and Release of Liability, fully understand its contents, and voluntarily agree to its terms.
